Anyone can register at the practice providing they live in the practice area (see the practice area map).
You need to complete a registration form (GPR1) or fill in the section on your medical card. You will also need to produce some form of photo ID (eg passport) and proof of address (eg recent utility bill). We prefer that you call into the surgery to register. This way a receptionist can give assistance with the forms and answer any queries or comments you may have. However, forms can be collected at the surgery and returned by post. If you have any difficulty registering please ask to speak to the reception manager who can give assistance.
When you register with the practice we will invite you to have a new registration appointment. Although you are not obliged to have this it is important for us to gather some essential information with regard to your health, and take details of any regular medications you are prescribed. It also gives you the opportunity to discuss any health problems and ask any questions and gives you a chance to meet the doctor or nurse face to face. It is only a 10-minute appointment.
When you register with the practice, you have the right to express a preference of doctor.
