The Practice Nursing team run clinics for the management and review of patients’ with Chronic Disease. This includes Diabetes, Asthma, COPD, Hypertension, Stroke, Coronary Heart Disease (CHD), Chronic Kidney Disease (CKD), and Peripheral Arterial Disease (PAD).
If you suffer from any of these conditions you will receive a letter or text message annually on your month of birth to invite you for your annual review. Some of the annual reviews will require you to follow the steps below and completing the online form and/or attending for a blood test prior to being seen by the Practice Nurse. You will be informed of this in your invite letter or text message. The reception team will advise you of the appropriate appointments to make when you contact us.
Our Chronic Disease Clinics run at various times throughout the week therefore we can accommodate the time that suits you best. If you have any concerns and feel you cannot wait until your annual review or are unable to complete the online form you can simply book an appointment by contacting reception.
If you do not wish to attend for your review, please let reception know, preferably in writing. We will not invite you again for 12 months but you will be invited annually for checks.
Asthma Review
Please note, any patient under the age of 18 years should not complete the steps below and instead arrange a face to face appointment with the Practice Nurse.
