If you are registering with the practice we invite you to have a new registration appointment. Although you are not obliged to have this it is important for us to gather some essential information with regard to your health, and take details of any regular medications you are prescribed. It also gives you the opportunity to discuss any health problems and ask any questions and gives you a chance to meet the doctor or nurse face to face. It is only a 10-minute appointment.
We do not speak to anyone else in the household about your care or visit to the Surgery when we phone, this is to protect confidentiality. We do not say where we are calling from. Your helpfullness, understanding and co-operation would be appreciated when we try to contact you by telephone,
Unless we have your consent recorded on your records we cannot discuss your appointments or health care with anyone else. Please take responsibility for your own health care and connunications.
We ask that you tell the Receptionists why you need to see the Doctor when they ask, they can make sure you get seen by the right person, at the right time and can signpost you to other services that may be more appropriate for you. If however you don't feel this is appropriate we will undestand.
PLEASE TRY SELF CARE WHEN APPROPRIATE BEFORE MAKING AN APPOINTMENT WITH THE DOCTOR. YOUR LOCAL PHARMACY AND NHS INFORM ARE A GOOD SOURCE OF HEALTH INFORMATION.
